docs/ is gitignored; startup applies migrations #326

The repository's docs/ folder is in .gitignore, so files there stay local; shared documentation belongs in Conderis pages. Program.cs runs MigrateAsync on every start (SQL Server), so a deploy applies pending migrations; --initialize-only runs only that step.
